Once
a Medicaid member with Preferred Medical Plan (PMP), your Primary Care
Doctor will provide or arrange for all of your medical care from the network of
contracted providers. PMP's goal is to arrange the Covered
Services you and your family need to help you stay healthy. You
play a key role by taking care of yourself and following the advice of
your Primary Care Doctor and other health care providers.
PMP
covers the same covered health care services as Medicaid and this
includes medical care for illness, injury, preventive health care and
wellness. There are no co-payments for services covered by PMP,
including your extra benefits.

Physician
Services includes the care and
treatment you get from Physicians, Advanced Registered Nurse
Practitioners (ARNP), Physician Assistants (PA), podiatrists (foot
doctor), chiropractors (back doctor), dermatologists (skin doctor), OB/GYNs
(obstetrician/gynecologists), county public health departments (CPHD),
midwives, certified nurse midwives, doctors at birthing center services,
doctors at ambulatory surgical centers and hospitals.

Inpatient
Hospital Services are the medical supplies and equipment,
therapy services, diagnosing services, medicines, room, board, and
nursing care you get while in the hospital. This also includes
medical care or treatment for your mental health.

Outpatient
Hospital Services includes the emergency room, dressings,
supplies, any treatment ordered by the doctor, and medical procedures
done as an outpatient at a hospital. Services like outpatient
dental surgery is also covered by PMP.

Child
Health Check-Up Services are regular and scheduled preventive
physical exams to find and treat medical conditions in children and
young people under 21 years of age. Doctors keep track of this
health screening physical exam for you and your children. A letter
will be mailed to you when it is time to schedule a visit.

Durable
Medical Equipment like wheelchairs, walkers, and special beds
are covered for PMP members. If you are in need of home health
services and/or durable medical equipment, you must talk to your doctor
for a referral.

Family
Planning Services includes planning, referral and counseling
services, doctor visits, lab testing, birth control medicine (including
implants), and supplies.

Hearing
Services include hearing tests and hearing aid services. |
|

Home
Health Services are nursing services that you get at home
from a registered nurse (RN), licensed practical nurse (LPN), or
licensed home health aide (HHA) and includes medical supplies,
appliances, physical, speech, and occupational therapy.

Independent
Lab Tests and Portable X-Ray Services are testing services
used to help diagnose medical problems that you may get at a doctor's
office, a hospital or lab.

Prescribed
Drug Services to provide prescribed drugs (medicines)
including food supplements from any pharmacy (drug store) in the network. All you need is a written prescription from your doctor.

Transportation
(Rides) is provided by PMP for non-emergency visits to your
doctor in Miami-Dade county, at no cost.
|
|
 |
Visual
Services include eye exam office visits, eyeglasses and
contact lens for PMP children and adults with a referral from your
doctor.
